Plan de Cuidados de Enfermeria A Pacientes con Delirium en la Unidad Cuidados Intensivos de la Clinica Santa Ana, en el 2022

La presente investigación centró su atención en la implementación de un plan de cuidados de enfermería en los pacientes con Delirium mediante los diagnósticos NANDA, NIC, NOC de la clínica santa Ana Cúcuta, Norte de Santander 2022. Para tal fin se propuso una metodología de enfoque cuantitativo, tipo explicativo y método inductivo. La técnica de recolección de datos consistió en una entrevista Delirium como proceso de sistematización del plan de cuidado para el manejo del paciente adulto con delirium bajo el Método: Escala de CAM ICU - Herramientas NIC y NOC. La población participante estuvo conformada por 12 pacientes de la Unidad de Cuidados Intensivos de la clínica. Como resultados de la investigación se encontró predominancia del delirium hiperactivo con un 41,6% sobre el delirium hipoactivo (33,3%). En cuanto a los pacientes con delirium hipoactivo y posterior al diagnóstico inicial, el 75% de los pacientes presentaron comprometido el sistema nervioso. Por otra parte, el 60% de los pacientes con delirium hiperactivo, tuvieron como diagnóstico de ingreso una etiología. En cuanto a los diagnósticos NANDA, la patología de mayor frecuencia fue confusión aguda 100%, patrón respiratorio ineficaz 41% y dolor agudo 16%. asimismo, y aplicadas las intervenciones del plan de enfermería se encontró que, a las 48 horas, la totalidad de los pacientes persistía con delirium mientras que, a las 72 horas, el 58% de los pacientes mejoraron esta condición. Finalmente, a través de la aplicación del plan de enfermería se logró que el 63 % de los pacientes mejorarán el puntaje NOC para confusión aguda a 4/5.

The present research focused its attention on the implementation of a nursing care plan for patients with Delirium trough NANDA diagnostics, NIC, NOC, of the Santa Ana Cúcuta clinic, in Norte de Santander, Colombia 2022. For this purpose, a methodology of quantitative approach, explanatory type and method was proposed. The data collection technique consisted of a Delirium interview as a systematization process of the care plan for the management of the adult patient with Delirum using the method: Scale of CAM ICU- tools NIC and NOC. The participating population consisted of 12 patients from the care unit. As research results found predominance of Hyperactive Delirium with a 41,6% about Hipoactive Delirium with 33,3%. In respect of the patients with Hipoactive Delirium and after the initial diagnostic, the 75% of the patients presented troubles their nervous system. In other side, the 60% of patients with Hyperactive Delirium, had an etiology as a diagnosis of admission. In respect of NANDA diagnoses, the most frequent pathology was acute confusion 100%, ineffective breathing pattern 41% and acute pain 16%, also and implemented the interventions of the nursing plan was found that at 48 hours, all patients persisted with Delirium while at 72 hours, the 58% of the patients improve that condition. Finally, through the application of the nursing plan it was possible that the 63% of patients improve their NOC score for acute confusional to 4/5
