dc.description.abstract | El Fontan es una cirugía paliativa y su fisiología establece efectos deletéreos tanto en la circulación pulmonar como en la sistémica, lo cual implica un deterioro en la clase funcional a largo plazo. El principal problema en los pacientes con cardiopatía congénita univentricular es la presión que tiene que soportar el ventrículo único por el volumen de los dos ventrículos. Lasresistencias vasculares pulmonares >2 ± 0.8 WU× m2 e índices cardiacos bajos son predictores del pronóstico después del procedimiento de Fontan. El objetivo de este estudio fue evaluar la respuesta de pacientescon cardiopatías congénitas con fisiología univentricular, la repercusión de los datos de resistencia vascular pulmonar(RVP)encontrados en el cateterismo prequirúrgico y los desenlaces evidenciados en el postoperatorio de Fontan. Se realizó una revisión de 61 historias clínicas de pacientes sometidos a Fontan durante los años 2010-2020 en la Fundación Cardiovascular de Colombia. Se encontró que 62% eran de sexo masculinoy50% se encontraban entre los 3 y 6 años. 84,5% de los pacientes provenían de Colombia y el 24% de Santander. El estado nutricional del 44,8% fue adecuado. La mediana de saturación fue 82% (RIQ 77-85%)y50% tenían insuficiencia leve de la válvula auriculoventricular. La mediana de la función ventricular fue62% (RIQ 58-68%),de RVP fue 1.68 unidades Wood·m2y de RVS fue12 unidades Wood·m2. La mediana de la presión de fin de diástole fue10 mmHg y de la presión media de la arteria pulmonar fue 12 mmHg. La complicación posquirúrgicamás frecuentefue el derrame pleural persistente (43,1%), seguido de la disfunción ventricular (17,2%). La mortalidad intrahospitalaria fue de 12,1%. dc.description.abstract | The Fontan is a palliative surgery and itsphysiology establishes deleterious effects both in the pulmonary and systemic circulation, which implies a long-term deterioration in the functional class. The main problem in patients with univentricular congenital heartdisease is the pressure that the single ventricle has to withstand due to the volume of the two ventricles. Thepulmonary vascular resistance >2 ± 0.8 WU× m2, and low cardiac indices are important predictors that influence prognosis after the Fontan procedure.The aim of this study was toevaluate the response of patients with congenital heart disease with univentricular physiology, the repercussion of the pulmonary vascular resistance(PVR)data found in the preoperative catheterization and the outcomes evidenced in the Fontan postoperative period. A review of 61 medical records of patients undergoing Fontan during the years 2010-2020 at the Fundación Cardiovascular de Colombia was carried out. It was found that 62% were maleand50% of the patients were between 3 and 6 years old. 84.5% ofthe patients were from Colombia and 24% were from Santander. The nutritional status of 44.8% was adequate when undergoing surgery. The median saturation was 82% (IQR 77-85%), and50% had mild atrioventricular valve insufficiency.
