Análisis Molecular de Metalobetalactamasa (blaVIM-2) y Betalactamasa (blaSHV-2) en Patotipos de Escherichia coli y Escherichia coli Comensales, Aislados de Niños del Área Metropolitana de Bucaramanga

La enfermedad diarreica aguda, es un problema de salud pública en niños, con una mortalidad anual de 5 a 6 millones de muertes tiene como principal agente causal bacteriano a E. coli, con la capacidad de adquirir genes (blaSHV-2 y blaVIM-2) codificantes de enzimas Betalactamasas que generan resistencia a medicamentos. Se planteó identificar molecularmente la presencia o ausencia de genes de resistencia para betalactamasas (blaSHV-2), metalobetalactamasas (blaVIM-2), en aislados clínicos de Escherichia coli asociadas a la enfermedad diarreica aguda (EDA) en población infantil de Bucaramanga y su Área metropolitana. Se estudiaron 86 cepas a las cuales se les realizo ensayos fenotípicos (Antibiograma y test de sinergia de doble disco) y genotípicos (Reacción en Cadena de la Polimerasa Simple). Se determinó que en las 86 cepas los antibióticos que presentaron mayor resistencia fueron Amoxicilina y Ampicilina con un 95% y el de menor resistencia fue Meropenem con un 5%. Genotípicamente se encontró que el 54,7% poseen el gen blaSHV-2 y el 38,4% tienen el gen blaVIM-2 y la mayor prevalencia por patotipos en el gen blaSHV-2 fue de ECEH (55%), ECET (37%) y ECAD (36%) y del gen blaVIM-2, ECEP (20%) y ECEI (18%). El presente estudio posee la mayor tasa de prevalencia de los genes blaSHV-2 y blaVIM-2 de Colombia en los patotipos y cepas comensales de E. coli.

Acute diarrheal disease is a public health problem in children, with an annual mortality of 5 to 6 million deaths, its main bacterial causal agent being E. coli, with the ability to acquire genes (blaSHV-2 and blaVIM-2) encoding Betalactamase enzymes that generate drug resistance. It was proposed to molecularly identify the presence or absence of resistance genes for beta-lactamases (blaSHV-2), metallobetalactamases (blaVIM-2), in clinical isolates of Escherichia coli associated with acute diarrheal disease (ADD) in a child population of Bucaramanga and its Area metropolitan. Eighty-six strains were studied to which phenotypic tests (Antibiogram and double disk synergy test) and genotypic tests (Simple Polymerase Chain Reaction) were performed. It was determined that in the 86 strains the antibiotics with the highest resistance were Amoxicillin and Ampicillin with 95% and the one with the least resistance was Meropenem with 5%. Genotypically, it was found that 54.7% have the blaSHV-2 gene and 38.4% have the blaVIM-2 gene and the highest prevalence of pathotypes in the blaSHV-2 gene was EHEC (55%), ECET (37 %) and ECAD (36%) and of the blaVIM-2 gene, ECEP (20%) and ECEI (18%). The present study has the highest prevalence rate of the blaSHV-2 and blaVIM-2 genes from Colombia in pathotypes and commensal strains of E. coli.
