Efecto de la implementación del MIPRES en los costos de salud de los afiliados de una EPS del nororiente colombiano entre abril y julio de 2017.

El incremento exagerado de los recobros al Fosyga por los servicios no incluidos en el plan de beneficios con cargo a la Unidad de Pago por Capitación (UPC), es una de las grandes dificultades por las que atraviesa el Sistema de salud colombiano, esto obligó al Gobierno a promulgar la resolución No. 1328 de 2016 mediante la cual crea una herramienta tecnológica de uso obligatorio para los profesionales, que permitiera obtener información veraz para la toma de decisiones. Teniendo en cuenta el alcance de la norma, se planteó la siguiente pregunta de investigación: ¿cuál es el efecto de la implementación del MIPRES sobre las frecuencias de uso y los costos de salud de los afiliados de una EPS Contributiva de la región Nororiente del país entre abril y julio de 2017? Para llegar a estos resultados se compararon los requerimientos de los servicios del año 2016 y el primer cuatrimestre posterior a la implementación del MIPRES, encontrando una disminución del 10% en el promedio mes de Ordenes Autorizadas, un 8% en cantidad total de servicios y un 6% en el costo promedio mensual, estos hallazgos pueden deberse a las dificultades propias del proceso de transición entre el anterior modelo de prescripción (CTC) y la implementación del MIPRES. Los medicamentos que más se prescribieron en el año 2016, aumentaron en un 72% su uso posterior a la implementación de MIPRES, lo que justificaría un análisis por parte del ministerio frente a la necesidad de incluirlos en el plan de beneficios. Si bien el MIPRES ha permitido tener la casuística a nivel nacional no ha representado un descenso importante en los costos por estas tecnologías no pos y por el contrario el alto número de devoluciones y anulaciones en el sistema ha dificultado el acceso al usuario al uso de estas tecnologías.

The exaggerated increase in the FOSYGA for services not included in the benefit plan from the Capitation Payment Unit (UPC), is one of the great difficulties that goes through the Colombian health system, this forced the government to promulgate the resolution No. 1328 of 2016 by which it creates a technological tool of compulsory use for professionals, which allows to obtain truthful information for the decision-making. Taking into account the scope of the standard, the following question of inquiry was raised: What is the effect of the implementation of MIPRES on the frequencies of use and the health costs of the members of a Contributive EPS of the Northeast region of the country between April and July of 2017? To reach these results were compared the requirements of the services of the year 2016 and the first quarter after the implementation of the MIPRES, finding a decrease of 10% in the average month of authorized orders, 8% in the total amount of services and 6% in the average monthly cost, results that may be due to the difficulties inherent in the transition process between the previous prescription model (CTC) and the implementation of the MIPRES. The drugs that were prescribed most in the year 2016, increased by 72% their use after the implementation of MIPRES, which would justify an analysis by the Ministry against the need to include them in the plan of benefits. Although the MIPRES has allowed to have the case at the national level has not represented a significant decrease in the costs for these No-POS technologies and on the contrary the high number of returns and cancellations in the system has hindered the access to the user to the use of these technologies.
