Generally avoidable maternal death has been considered the highest expression of inequality in health, in which primarily women with greater socioeconomic difficulties are affected, and in which there is a great difference in the mortality rates presented by developed countries compared to developing countries. The motivation of this research is to present the deaths of pregnant women in the department of Santander, Colombia, during the years 2012 to 2015, through a systematic review of the units of analysis provided by the Departmental Health Secretariat carried out in Santander, in order to offer input that allows the proposal of actions that would contribute to diminish these fatal outcomes in our territory. The present investigation was a descriptive, retrospective cross-sectional study in which the characteristics of maternal mortality were determined in pregnant or postpartum women who underwent complete analyzes of health care and which were not caused by external causes or violence. It was identified that the most frequent delay in the 49 cases of maternal deaths was type IV, which refers to the delay in receiving adequate and timely medical treatment (87.8%), mainly due to deficiencies in promotion and prevention services (63.2%). The majority of the deaths were avoidable (61.2%), in the postpartum period (84%) and in users of the subsidized regime (57.1%).
